To be considered, your package must have:

  1. Signed and sealed reference letter
  2. Transcript
  3. Specific learning disability documentation
  4. Completed and signed PCF application
  5. Photo

Step 1:

  • Request a reference letter from a school counselor, teacher, principal or extracurricular activity leader
  • This letter must be signed and in a sealed envelope

Step 3: 

  • Visit your high school’s registrar office
  • Request a Freshman through Senior year transcript 
  • Unofficial is accepted

Step 4:

  • Obtain your Specific Learning Disability (SLD) documentation
  • This includes individualized education programs (IEP), 504 plans or a letter from your psychologist or the institution who performed your testing 

Step 5: 

  • Click the PCF application below button
  • Download the PDF
  • Use a computer and type out all the required information
  • Print your completed application
  • Sign it

Step 6:

  • Print a recent pic of yourself or find a current photo to include

Step 7:

  • We do not accept applications via email!
  • Paperclip all items (don’t staple)
  • Ship 1 package with USPS Priority Mail, FedEx or UPS so it can be tracked
  • Mailing address: 5758 Geary Blvd. #164, San Francisco, CA 94121
  • PCF is not responsible for lost or misaddressed packages
  • Write your return address on the package; if it gets lost you’ll get it back and your private info. will be secure
  • Package must be post-marked on or before April 14, 2018
